At every new appointment, in the emergency department or while you are away from home, you will be asked the same things: what your diagnosis is, what operation you had and when, what medicines you take, what you are allergic to. This page helps you write it all down once and get a printable card you can keep in your wallet or your bag.

Everything you fill in stays in the browser on this device. There is no server, there is no account, nothing is sent anywhere. The PDF is generated locally, on your own computer or phone. Saving to the device is optional and you can delete it at any time.

How to use it

  • Print it and fold it in half. It fits in a wallet or in an inside jacket pocket.
  • Keep a copy at home, somewhere visible, where someone else can find it if needed.
  • Show it at every new appointment, in the emergency department and when you travel.
  • Make a new one after every important change — another operation, stoma reversal, a change of medication.

The card does not replace official medical documents. Also carry your discharge letter or your medical summary with you when you go to an important appointment.
